    The pharmacist will be working in the Home Pharmacy that supports both adult and pediatric hospitals. This role involves providing home infusion pharmacy services, enteral therapy, and other specialized injectable therapies for pediatric patients and adults with childhood diseases such as cystic fibrosis and hemophilia. Each day, the primary goal is to address patient needs before discharge from the hospital, ensuring they receive proper treatment at home. The pharmacist will work closely with chemotherapy, hydration antibiotics, and medications to assist in patient treatment.

    Responsibilities

    + Fulfill patient needs before hospital discharge to ensure proper home treatment.

    + Work closely with chemotherapy, hydration antibiotics, and medications.

    + Clean up the database and manage provider registry and templates.

    + Provide coverage for existing staff, including morning rounds with nurses.

    + Conduct patient follow-ups and manage assigned shifts for the day.

    + Set up refills and make phone calls to patients.

    + Collaborate with the intake, delivery, and IV room teams.

    Essential Skills

    + Pharmacist License

    + 3-5 years of experience, preferably in inpatient or home infusion settings

    + High understanding of medications and their treatments

    + Strong attention to detail and analytical skills

    + Excellent customer service skills

    Additional Skills & Qualifications

    + Cpht certification

    Work Environment

    The Home Pharmacy is located offsite from the main hospitals in Menlo Park. The onsite team includes the billing, intake, fulfillment, and delivery teams, all in one location. The team has a great culture with a strong bond and is a smaller-sized but very close-knit team. The work shift is Monday to Friday with varying morning hours.

    This is a Contract position based out of Menlo Park, CA.

    Pay and Benefits

    The pay range for this position is $70.00 - $75.00/hr.
