    The University of Miami Department of Psychiatry has been contracted by the Florida Department of Health to provide behavioral health and psychiatric consultation services to selected pediatric primary care practices in the community.

     

    The behavioral health hub will provide mental and behavioral health expertise and support to the pediatric site through comprehensive practice transformation services. This includes trainings, technical assistance, telepsychiatry consults (eg. psychiatrists and psychologists), and care coordination/management to advance quality improvement, patient-centered care, and characteristics of high performing primary care including the integration of Behavioral Health Services.

     

    The Social Worker LCSW will be a key contact for a Florida Department of Health behavioral health grant. The incumbent will have an integral part in the program development under the Program Director and will be responsible for the adherence to the contract deliverables. The incumbent will also serve as the liaison with pediatric primary care centers, behavioral health organization, and other mental health stakeholders in the community and with the State of Florida.

     

    The Social Worker LCSW will be responsible for participating in FLDOH collaborative meetings, meetings with potential pediatric providers, and meetings to network with behavioral health organizations. He/she will take the lead in increasing referrals, triaging/conducting intakes, and conducting provider-to-provider consults. The incumbent will assist patients and their families to follow treatment recommendations. He/she will also advocate for access to resources by partnering with behavioral health organizations and ensuring appropriate agreements are in place. The incumbent will provide input in regular training topics for the UM Behavioral Health Hub.

     

    + Monitor referrals via the BHH inbox ([email protected]) on a daily basis.

    + Follow up BHH referrals and initiate social services assessments including GOT Transition Assessment Survey and PSC-17 assessment forms.

     

    BHH Tasks and State Deliverables

    BHH Development and Maintenance:

    + Develop and maintain the BHH to provide Behavioral Health Hub services to participants throughout the contract term.

    Access to Resources:

    + Provide access to additional behavioral health resources as necessary to maintain and support the delivery of behavioral health services to clients.

    Medical Records Management:

    + Create and maintain medical records for all clients receiving BHH services through the contract requirement, ensuring compliance with confidentiality and regulatory standards.

    Enrollment Agreements:

    + Enter into agreements with enrollees to formalize participation in BHH services.

    Provider Maintenance:

    + Maintain relationships with participating providers to ensure the minimum number of client referrals as specified in the contract.

    Quarterly Participant Roster:

    + Complete the BHH participant Roster each quarter and submit it as required.

    Evidence-Based Training:

    + Conduct at least one evidence-based skill-building training each quarter for enrollees to enhance their competencies.

    Skills Assessment Administration:

    + Administer and re-administer the Enrolled Provider Skills Assessment Tool for each enrollee three times during the contract year and analyze the results using the Enrolled Provider Skills Assessment Summary Template.

    Consultation Services:

    + Provide consultation services to enrollees, offering guidance and support related to their behavioral health needs.

    Care coordination:

    + Deliver care coordination services to clients, ensuring seamless access to necessary services and resources.

    Client Satisfaction Assessment:

    + Determine each enrollee’s satisfaction with the BHH’s services by administering the Primary Care Provider (PCP) Satisfaction Survey, collecting completed surveys, and analyzing the results to complete the BHH Participant Satisfaction Summary Report.

    Transformation Plans:

    + Develop, implement, and update a Transformation Plan for each enrollee based on the results of their skills assessments within 150 calendar days of enrollment.

    Client Referral Metrics:

    + Ensure a minimum of 60 clients are referred to the BHH each quarter, completing the Client Roster for each referral and submitting it with the Quarterly Progress Report.

    Client Satisfaction Surveys reviewed with Program Manager:

    + Measure client satisfaction with BHH services by providing a Client Satisfaction Survey one month after referral and every six months or prior to discharge.

    Quarterly Reporting:

    + Complete the Statewide Pediatric Behavioral Health Collaborative Summary Report Template and the Quarterly BHH Data Report Template each quarter.

    Quality Improvement Initiatives:

    + Identify, design, and implement a BHH quality improvement initiative related to pediatric behavioral health hub.

    Statewide Initiative Participation:

    + Collaborate with the Department and other Regional BHHs to identify and participate in the implementation of one statewide quality improvement initiative.

    Annual Data Reporting:

    + Consolidate data from the Quarterly BHH Data Report and use it to complete the Annual BHH Data Report.

    Data Submission:

    + Provide data to the Department’s designated third-party contractor electronically as directed, documenting the date and method of submission in the Quarterly Progress Report.

    Ongoing Responsibilities:

    + Maintain ongoing responsibilities related to client care, quality improvement initiatives, and compliance with contractual obligations throughout the contract term.

     

    Department Specific Qualifications

    Education:

    Master’s Degree in Social Work or in a relevant mental health field

    Certification and Licensing:

    Active License

    Experience:

    Minimum one year at a behavioral health organization or academic institution. Experience in working with children and families is preferred.
