Description:
Position Purpose:
Provides personal care to patients receiving services by MercyOne North Iowa Hospice.
What you will do:
+ Effective oral and written communication includingtimelydocumentation with all interdisciplinary team members and reports significant changes inpatient’scondition to registerednurse.
+ Completion ofcare plan documentationin a timely manner.
+ Provides temporary relief topatient’sfamily when in the home setting/nursing facility.
+ Participates in regularly scheduled meetings and in-services andmaintainscompetencies.
+ Collaborates with peers to complete weekly/monthly scheduling andfollowsguidelines.
+ Organizes and prioritizes workload within assigned shift/caseload, including the ability todemonstrateflexibility.
+ Demonstrate theability to work with frequent interruptionsin order tomeet patient/family needs acrossMercyOneNorth IowaHospice.
+ Utilizes Hospice resources efficiently.

Minimum Qualifications:
Education:
Certification from a Certified Nurse’s Aide (CNA) program through classroom and supervised practical training totaling at least 75 hours with 16 hours devoted to supervised practical training, must have a minimum of 12 hours of in-service per year to maintain CNA Certification
OR
Currently enrolled in an ADN, BSN, diploma program with successful completion of first semester of ADN program, OR first year completion of nursing courses in BSN or Diploma program, OR graduate of an ADN, Diploma, BSN or LPN program.
Must pass Hospice competency requirements Supervised by a licensed nurse.
Experience:
Aide experience strongly preferred.

Ability to successfully complete competency testing
Ability to read, write and carry out directions and accept supervision required.
Empathetic attitude toward the care of the sick and dying and their families.
Demonstrates a commitment to individual development.
Ability to work independently yet within a team structure.
Competent or willing to learn and develop competence with computer skills and programs and access in a timely manner.
Ability to be flexible, do a variety of duties and to prioritize with frequent interruptions.
Must be able to read, write and/or speak English.
