    Licensed Practical Nurse (LPN) – Case Manager

     

    Schedule: Monday–Friday, 9:00 AM–5:00 PM

    Pay:

    + Local: $47/hour

    + Mileage: $0.70/mile

     

    Assignment: 13-week contract to start (Temp-to-Perm and Direct Placement options available)

    About the Role

    Amergis Staffing is partnering with a trusted Adult Day Health organization dedicated to helping older adults and individuals with disabilities remain safe and independent at home. Through personalized care plans and strong clinical oversight, this team enhances quality of life for participants and their caregivers.

     

    As an LPN Case Manager, you’ll play a vital role in assessing health needs, implementing care plans, and coordinating resources to ensure participants receive exceptional care. This position combines clinical expertise with compassionate outreach in a community-based setting.

    What You’ll Do

    + Conduct monthly in-home visits to provide education, skills training, and assess health/environmental changes.

    + Perform semi-annual reviews of care plans, goals, and medication reconciliation.

    + Address concerns related to home safety, psychosocial well-being, and physical health changes.

    + Collaborate with multidisciplinary teams and communicate with PCPs and other providers.

    + Complete all documentation per regulatory standards and maintain accurate patient records.

    + Mentor team members and contribute to a supportive, inclusive work environment.

    + Uphold cultural humility and compassion in all interactions.

    Qualifications

    + Active Massachusetts LPN license in good standing.

    + 2+ years of nursing experience preferred (new grads welcome).

    + Strong clinical assessment and decision-making skills.

    + Proficiency with Microsoft Office and EMR systems.

    + Ability to travel to member homes; valid driver’s license required.

    + Prior care management experience a plus.

    Additional Details

    + Work primarily in residential/community settings; occasional remote work possible.

    + Member-facing role with in-person client interactions.

    + Contract requirements: TB test, physical exam.
