    Weekend Case Manager RN

     

    Schedule: Saturday–Sunday, 9:00 AM–5:00 PM (paid for 24 hours)

    Pay:

    + Local: $57/hour

    + Mileage reimbursed: $0.70/mile

     

    Duration: 13 weeks to start (Temp-to-Perm + DP options available)

    Position Summary

    Amergis Staffing is partnered with a much respected Adult Day Health organization providing community-based care services that help elderly and disabled individuals remain safely and comfortably in their homes. Their programs support clients and caregivers through customized care plans, clinical oversight, and ongoing case management.

     

    The Weekend Case Manager RN (Baylor RN) functions independently during weekend operations, delivering high-quality, person-centered home health care throughout Lawrence and Lowell. This role ensures clients remain safe, stable, and supported while helping to reduce preventable hospitalizations.

    Key Responsibilities

    + Independently manage weekend Baylor shifts (16 hours worked, paid for 24).

    + Complete intake, annual assessments, and admission visits.

    + Complete all required documentation, including client assessments and multidisciplinary plans of care; coordinate with PCPs and other healthcare providers.

    + Review and educate caregivers on admission binder materials, communication tools, and/or EVV systems.

    + Conduct regular in-home visits, provide caregiver skills training, and assess for health or environmental changes.

    + Conduct semi-annual visits, updating care plans, goals, medication reviews and reconciliations, and caregiver communication tool audits.

    + Identify concerns related to safety, psychosocial status, and physical health.

    + Participate in multidisciplinary team meetings and collaborate to support client needs.

    + Promote a culture of customer service, compassion, and quality care.

    + Serve as a brand ambassador for Vitra Health, reflecting its mission, values, and commitment to diversity.

    + Other duties as assigned.

    Qualifications

    + Active Massachusetts RN license in good standing.

    + 2+ years of nursing experience preferred.

    + Required experience in clinical assessments.

    + Strong clinical decision-making and care delivery skills.

    + Intermediate–advanced proficiency with Microsoft Office Suite.

    + Willingness to utilize telehealth tools (video, chat, etc.) when appropriate.

    + Electronic medical record experience.

    + Care Management experience.

    Working Conditions

    + Work performed in residential and community settings.

    + Must travel to member homes; occasional remote work possible.

    + Valid driver’s license with no restrictions; must be mobile across Massachusetts.

    + Member-Facing: YES – in-person client interactions required.
