    Position Summary:

    Nurse Navigator facilitates all stages of the patient referral process.

     

    Works in direct coordination and cooperation with referring physicians, specialists, and self-referring patients. The Nurse Navigator is the resource/main contact for patients during the initial consultation phase.

    Essential Functions and Responsibilities:

    Facilitates all stages of patient referral process

     

    2.

     

    Maintains operational responsibility for navigation program and development.

     

    3.

     

    Ensures proper scheduling of all appointments with specialists

    _Required:_

    * Licensed Registered Nurse with Bachelors Degree in Nursing

    * Oncology Certified Nurse within 18 months of transfer/hire
