Overview
LTC ACO is a national value-based care organization dedicated to improving outcomes for
long-term care residents through population health, proactive clinical management, and
strong partnerships with post-acute and long-term providers. We support physicians,
medical groups, and facilities participating in the Medicare Shared Savings Program (MSSP)
by delivering analytics, technology-enabled workflows, and clinical programs that reduce
unnecessary hospitalizations, improve quality, and advance the patient and facility
experience.
The Director of Clinical Innovation is a new leadership role responsible for driving
enterprise-wide transformation initiatives, advancing data-driven clinical programs, and
building scalable systems that support high-quality, efficient care for long-term care
residents. This role will lead cross-functional projects across Clinical Programs, Quality,
Technology, Analytics, and Account Management to develop and implement high-impact
initiatives that advance LTC ACO’s strategic goals.
This role offers the opportunity to shape the future of value-based care in long-term care.
You will help develop programs that improve outcomes and experiences for nursing home
residents nationwide while supporting physicians and clinical teams across the country.
Responsibilities
RESPONSIBILITIES/ACCOUNTABILITIES:
Enterprise-Wide Projects and Transformation Initiatives
• Lead design and execution of high-priority organizational initiatives that strengthen clinical, operational, and provider-facing performance.
• Serve as the owner/driver for major innovation efforts, ensuring disciplined execution, cross-functional alignment, and measurable outcomes.
• Develop new care models, workflows, and interventions tailored to long-term care providers and high-risk MSSP populations.
• Identify opportunities to streamline processes and introduce best practices across the enterprise.○ Complete course work and chapter specific course work toward CPC with the expectation of completing certification requirements within one year of hire.
Data Analytics & Customer Insights
• Partner with Analytics to translate claims data, EMR data, risk-stratification outputs, and provider-level insights into actionable program design.
• Use data to identify cost drivers, variation patterns, gaps in care, and opportunities for targeted clinical interventions.
• Develop dashboards, reporting tools, and insight briefs that guide clinical and operational decision-making.
• Facilitate provider and facility listening sessions, focus groups, and qualitative research to surface user insights that inform solution design.
Strategic Development
• Support executive leadership in setting ACO strategy by providing market insights,performance trends, and innovation recommendations.
• Partner with the Director of Clinical Programs to design new lines of service, pilots, and programs that improve clinical outcomes and total cost of care.
• Build business cases for new initiatives and ensure alignment with MSSP requirements and organizational priorities.
• Partner with Technology and Product to create digital workflows and tools that support clinical and provider-facing teams.
Expected Results
• Measurable improvements in key ACO metrics: total cost of care, hospitalizations, SNF utilization, quality scores, provider engagement, and workflow adoption.
• New clinical programs or workflows successfully piloted and scaled.
• A clear roadmap for innovation grounded in data, provider feedback, and organizational priorities.
• Strong cross-functional collaboration and efficient operational execution.
COMPLIANCE:
Complies with and promotes adherence to applicable legal requirements, standards,
policies and procedures including but not limited those within the Compliance and
Ethics Program, Standard/Code of Conduct, Federal False Claims Act and HIPAA.
Qualifications
• Required:
• Bachelor’s Degree and 5+ years of experience in value-based care, population health, healthcare operations, clinical program design, quality improvement, or related fields.
• Experience working in an ACO, health plan, provider group, post-acute organization, or similar care delivery environment.
• Demonstrated ability to lead cross-functional projects and deliver results.
• Strong analytic literacy and experience using claims, EMR, or risk-stratification data to drive decision-making.
• Excellent communication skills with the ability to synthesize complex information into clear, actionable insights.
• Experience designing or improving clinical programs, workflows, or operational processes.
• Preferred:
• Advanced degree (MPH, MBA, MS, MHA, MSN, etc.).
• Experience in long-term care, geriatrics, or high-risk populations.
• Demonstrated ability to innovate and scale programs across distributed provider groups.
• Familiarity with Lean, Six Sigma, PDSA cycles, or other improvement
methodologies.

Restrictions apply based on collective bargaining agreements, applicable state law and factors such as pay classification, job grade, location, and length of service.
Posted Salary Range
USD $145,000.00 - USD $145,000.00 /Yr.
