Clinical Admissions RN - Full TimeLocation: In-home assessments in Cambridge, Arlington, Somerville, East Boston, Newton, Wellesley, and Needham, MASchedule: Monday–Friday, 9:00 AM–5:00 PMPay:• Local: $57/hour• Mileage: $0.70/mile reimbursementDuration: 13 weeks to start (Temp-to-Perm & Direct Hire options available)________________________________________About the RoleAmergis Staffing is partnering with a respected Adult Day Health organization dedicated to helping elderly and disabled individuals remain safe and comfortable in their homes. This program provides personalized care plans, clinical oversight, and ongoing case management to support both clients and caregivers.As an RN Case Manager, you’ll play a vital role in assessing, planning, implementing, and reevaluating care for clients. You’ll collaborate with an interdisciplinary Care Team to develop proactive action plans that ensure safety, stability, and quality of life.________________________________________Why You’ll Love This Role• Consistent Schedule: Monday–Friday, no nights or weekends.• Meaningful Impact: Help seniors and individuals live safely at home.• Autonomy & Support: Work independently while collaborating with a dedicated team.________________________________________Key Responsibilities• Complete intake, annual assessments, and admission visits.• Document client assessments and multidisciplinary care plans; coordinate with PCPs and providers.• Educate caregivers on admission materials and communication tools.• Conduct in-home visits, caregiver training, and health/environmental assessments.• Perform semi-annual visits: update care plans, review goals, reconcile medications, and audit caregiver tools.• Identify and address safety, psychosocial, and health concerns.• Participate in team meetings and collaborate to support client needs.• Promote a culture of compassion, quality care, and customer service.________________________________________Qualifications• Active Massachusetts RN license in good standing.• 2+ years of nursing experience preferred.• Clinical assessment experience required.• Strong clinical judgment and decision-making skills.• Proficiency with Microsoft Office Suite; comfort with telehealth tools.Preferred:• EMR experience.• Care management background.________________________________________Additional Details• Work occurs in residential and community settings.• Travel required; valid driver’s license needed.• Member-facing role with direct in-person care.• Contract requirements: TB test and physical.________________________________________Interested?Contact Adam Harrington at Amergis Healthcare Services:Call/Text: 781-400-8053Email: [email protected]
