Responsibilities:

+ Provide high-quality, individualized care to residents under the supervision of the Registered Nurse and/or Charge Nurse

+ Administer medications, treat wounds, and perform other medical procedures as needed

+ Develop and implement individualized care plans in collaboration with interdisciplinary team members

+ Maintain accurate and up-to-date records of resident care and treatment

+ Monitor resident vital signs and report any changes or concerns to licensed nursing staff

+ Collaborate with other healthcare professionals to ensure comprehensive care

+ Perform other duties as assigned by the Registered Nurse and/or Charge Nurse

Requirements:

+ Current LPN/LVN license in the State of New Jersey

+ 0 years of experience (new graduates welcome!)

+ Strong communication and interpersonal skills

+ Ability to work collaboratively as part of a team

+ Compassionate and empathetic approach to patient care

+ Ability to maintain confidentiality and adhere to HIPAA guidelines
