Job Summary and Qualifications
The Social Worker is part of the integrated Case Management Team who collaborates patients and their families to support patients coping with social, emotional, economic and environmental problems. The social worker works jointly with community and health staff on social matters that may affect present and future health needs of patients. The social worker may be called upon to act as a liaison for the medical and social issues that may arise and communicates information and recommendations to management as needed.
+ Performs ongoing monitoring of caseload for Case Manages based on appropriate level of care, utilization of services and plan of care. Ensures equitable caseload assignments.
+ Rounds with Case Managers and Medical Social Workers over course of day as assigned by Manager/Director.
+ First line resource to Case Managers, Utilization Review, and Medical Social Workers. Elevates issues as appropriate.
+ Monitors patient initial Discharge Planning Assessment and BPCI program to ensure timely intervention and reporting by Case Managers.
+ Maintains a partial caseload as volume dictates while maintaining Team Lead responsibilities and supervision.
+ When staffing necessitates will take full caseload/assignment.
+ Communicates potential/actual need for employee remediation to Manager and/or Director.
+ Collaborates with physicians and Health care team in planning the care of patients and families within the acute setting and along the healthcare continuum.
+ Involves the physician, patient and family in planning care and the discharge plan.
+ Utilizes discharge planning rounds with physicians and care conferences with staff to plan and coordinate care.
+ Monitors plan of care to ensure timely and cost-effective use of resources.
+ Monitors patient’s progress along the plan of care and intervenes appropriately on variances when identified.
+ Provides decision support to the admitting office, registration and nursing supervisor as well as physicians’
What Qualifications You Will Need:
+ Master of Social Work Degree required
+ A minimum of 5years experience in a healthcare setting Required
+ Licensed Clinical Social Worker or Licensed Master of Social Work Preferred
+ Case Management Certification Preferred
