Position Summary
The Social Work Manager is responsible for the overall clinical management of the social work and service coordinator team at Eddy SeniorCare PACE. This leadership role ensures high-quality participant care, drives program growth, and supports integrated care coordination across the continuum. The Social Work Manager serves as an educator, resource, and key member of the Leadership Team, contributing to enrollment growth and program expansion.
Key Responsibilities
+ Lead and supervise MSW and case management staff, ensuring competency and compliance with all regulatory standards.
+ Drive quality outcomes for participants through proactive care planning and risk management.
+ Recruit, hire, train, and evaluate social work team members; manage payroll and performance processes.
+ Facilitate interdisciplinary team meetings, morning huddles, and plan-of-care discussions.
+ Monitor participant satisfaction and implement strategies to improve quality of life and reduce avoidable hospitalizations.
+ Collaborate with leadership to grow program census and support new site development.
+ Maintain expertise in PACE model, integrated care, value-based care, and transitions of care.
+ Serve as a resource for community programs, payment sources, and social services.
+ Ensure compliance with federal, state, and organizational policies; participate in quality improvement initiatives.
+ Provide direct social work services as needed, including assessments, counseling, crisis intervention, and discharge planning.
Qualifications
+ **Education:** Master’s Degree in Social Work (MSW) required.
+ **Licensure:** Current NYS Social Work license required.
+ **Experience:** Minimum 1 year working with frail or elderly populations; leadership experience strongly preferred.
+ **Skills:** Strong leadership, communication, and organizational skills; proficiency with EMR systems and Microsoft Office; knowledge of Medicaid and community resources.
+ **Preferred:** Experience in PACE, integrated care coordination, and value-based care models.

Pay Range: $41.45 - $55.95
Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
