    Part-Time Clinical Assessment RN

     

    Schedule: Monday–Friday | 9:00 AM–5:00 PM

     

    Hours: 24–30 hours/week (3–4 days/week)

     

    Assignment: 13 weeks to start (Temp-to-Perm & Direct Placement options available)

    Compensation

    + Pay Rate: $57/hour

    + Mileage Reimbursement: $0.70/mile

    Position Overview

    Amergis Healthcare Staffing is partnering with a well-respected Adult Day Health organization dedicated to helping elderly adults and individuals with disabilities remain safe, stable, and independent in their homes. Through personalized care plans, strong clinical oversight, and dedicated case management, this team provides essential community-based support to clients and caregivers.

     

    As a Clinical Assessment RN, you will ensure client safety and continuity of care by conducting comprehensive in-home assessments, developing individualized plans of care, and collaborating with an interdisciplinary Care Team. This role plays a critical part in preventing avoidable hospitalizations and supporting long-term wellness.

    Key Responsibilities

    + Conduct intake, annual, and admission assessments in client homes.

    + Complete all required documentation, including assessments, multidisciplinary plans of care, and communications with PCPs and related providers.

    + Educate caregivers on admission materials, communication tools, and EVV/paper systems depending on program needs.

    + Perform regular RN home visits to assess changes in health status, environmental safety, and caregiver competency.

    + Complete semi-annual visits: update care plans, review goals, perform medication reconciliation, and audit caregiver communication tools.

    + Identify and address concerns related to physical health, psychosocial well-being, or home environment safety.

    + Participate in interdisciplinary team meetings to support coordinated and proactive care delivery.

    + Provide culturally responsive, empathetic care to diverse client populations.

    + Promote a culture of compassion, quality, and exceptional customer service.

    + Perform additional duties as assigned.

    Qualifications

    + Active Massachusetts RN license in good standing.

    + 2+ years of RN experience preferred.

    + Experience conducting clinical assessments required.

    + Strong clinical judgment, critical thinking, and communication skills.

    + Intermediate–advanced proficiency with Microsoft Office Suite.

    + Ability to utilize telehealth tools (video, chat, etc.) when appropriate.

    + Electronic medical record systems preferred

    + Care management or community-based nursing preferred

    Working Conditions

    + Work performed in residential and community settings.

    + Regular travel to client homes required; some remote documentation time possible.

    + Member-facing: Yes – direct, in-person client interactions required.

    + Valid driver’s license with the ability to travel throughout the region.
