As the Market Director of Case Management, you will lead market-wide case management operations in support of Ascension’s Mission to deliver compassionate, person-centered care. Partners with clinical and physician leaders to strengthen care coordination, ensure safe transitions, promote stewardship of resources, and support high-performing teams focused on quality, dignity, and service to all.

Responsibilities
Direct strategy of the Case Management (CM) program across the ministry markets.
+ Provide leadership to facility staff to identify, implement, or enhance CM programs consistent with the organization and market strategies.
+ Identify process improvement opportunities. Develop, implement, monitor, and revise action plans.
+ Disseminate information accurately and timely to multiple audiences. Lead regularly scheduled communication sessions with hospital CM directors.
+ Facilitate CM education within the market. Provide orientation and onboarding support to new hospital CM leaders.
+ Identify and propose opportunities to improve current or new technology to enhance performance and productivity.
+ Escalate issues to market and CM leadership as appropriate.
Requirements
Licensure / Certification / Registration:
+ Registered Nurse obtained prior to hire date or job transfer date required.
Education:
+ Bachelor's degree in nursing or diploma from an accredited school/college of nursing required.
+ Master's degree or combination of Bachelor's degree and experience preferred. OR 5 years of experience required.
+ 10 years of experience preferred.
+ 2 years of leadership or management experience required.
+ 5 years of leadership or management experience preferred.
Additional Preferences
+ Master’s degree in Health Administration, Business Administration (MBA), or related field
+ 10+ years of acute care Case Management experience
+ 5+ years of leadership experience managing Case Management teams in an acute care setting
+ Active Case Manager certification (ACMA or CCMC)
