    Job Description:

    Hospice Certified Nursing Assistant (CNA)

    **Location:** Grand Rapids

     

    Schedule:** Monday–Friday | Day Shift | **No weekends

    What You’ll Do

    + Deliver hands-on, compassionate care aligned with each patient’s individualized care plan

    + Support patients and families with dignity, comfort, and emotional presence during end-of-life care

    + Assist with personal care and educate patients and caregivers on self-care techniques

    + Communicate patient updates and changes in condition to the RN Case Manager and care team

    + Accurately document care provided and patient needs

    + Help prevent unnecessary hospitalizations by supporting continuity of care at home

    What We’re Looking For

    + High School Diploma or GED

    + At least 1 year of experience as a Nursing Assistant or Hospice Aide

    + Completion of a state-approved Hospice Aide training program with competency evaluation (per 42 CFR §484.36)

    + Hospice or home health experience preferred

    + Reliable transportation, valid driver’s license, and auto insurance (approximately 50% travel)

    + Ability to lift 50–100 lbs and perform hands-on caregiving duties

    + Preferably resides within the service area
