The Nurse Practitioner, Paneled applies advanced education and clinical competencies to achieve optimal patient panel outcomes. The Nurse Practitioner works on problems of diverse scope and complexity ranging from moderate to substantial.
The Nurse Practitioner, Paneled provides health promotion and maintenance through the diagnosis and treatment of acute illness and chronic conditions. Prescribes medication, examines patients, diagnoses illnesses, and provides treatment. Advises executives to develop functional strategies (often segment specific) on matters of significance. Exercises independent judgment and decision making on complex issues regarding job duties and related tasks, and works under minimal supervision. Uses independent judgment requiring analysis of variable factors and determining the best course of action. Serves primary care provider for their assigned patient panel in coordination with the patient care team, subject to state laws regarding scope of practice and physician supervision/collaboration

**Patient** **Care** **Responsibilities** :
+ Evaluatesandtreatscenterpatientsinaccordancewithstandardsofcare.
+ Followslevelofmedicalcareandqualityforpatientsandmonitorscare using available data and chart reviews.
+ Assistsinthecoordinationofpatientservices,includingbutnotlimitedto specialty referrals, hospital and SNF coordination, durable medical equipment and home health care.
+ Actsasanactiveparticipantandleadsthecareteamthroughdailyhuddles.
+ HelpsAssociateMedical Director (AMD), Physician and Center Administratorinsettingatoneofcooperationinpracticebydisplayinga professional and approachable demeanor.
+ Completesallmedicalrecorddocumentationinatimelymannerworking with a quality-basedcoder tooptimizecoding specificity.
+ FollowspolicyandprotocoldefinedbyClinicalLeadership.
+ MeetswithAMDaboutqualityofcare,reviewofoutcomedata,policy,procedureand records issues.
+ Participatesinpotentialgrowthopportunitiesforneworexistingservices within the Center.
+ Participatesinthelocalprimarycare“on-call”programofCenterWellSenior PrimaryCare.
+ Ensurespersonalcompliancewithlicensing,certification,andaccreditingbodies.
**In addition to the above responsibilities** , this role requires the Paneled Nurse Practitioner to take on the role as Primary Care Provider for their assigned patient panel in coordination with the patient care team, subject to state laws regarding scope of practice and physician supervision/collaboration. This includes but is not limited to the following additional responsibilities:
+ **Clinical Responsibility and Continuing Care for your** **P** **atient** **P** **anel** **:** accountableforcomprehensive, long-term care, ongoing treatment plans and coordination with specialists and other care providers.Responsibilityfor results review (imaging, laboratory testing, specialtyreferraland review of and follow through of recommendations from specialists).
+ **Commitment to Long-term Patient Care:** commit to managing the health ofyour patient panel over an extendedperiod. This will requireacute and chronic treatment plans and proactive managementthrough prevention and education.
+ **Accountab** **le** **for the clinical outcomes and total cost of care for assigned patient** **panel:** demonstratesproactive and sustained engagement in improving health outcomes,optimizingresourceutilization, and ensuring effective panel management.
+ **Focus on Quality and Improved Patient Care** **:** responsible, internally,forcompleting/closingall quality metrics (HEDIS, STARS, EDAPS)for their panel of patients.
+ **Participate and** **L** **ead** **H** **igh** **R** **isk** **Ro** **unds with** **C** **are** **T** **eam** **:** tailorcare plan to patient needs andensureappropriate utilization.
Required Qualifications
+ Minimum of two to five years directly applicable **experience in primary care, geriatrics, complex care, long-term care or similar** **.**
+ Successful completion of a nurse practitioner program with board certification.
+ Active, unrestricted nurse practitioner license in state of practice location.
+ Active and unrestricted DEA license.
+ Appropriatecertificationand credentialstowriteprescriptions **in accordance with** **applicable state and federal regulations governing Nurse Practitioners** **.**
+ Excellent communication skills, written and verbal;demonstratea high levelof skill with interpersonal relationships and communications with all colleagues; fully engaged in the concept of “Integrated team-based care”.
+ Willingness and ability to learn/adapt to practice in a value-based care setting.
+ Superior patient/customer service.
+ Basic computer skills, including email and EMR.
+ This role is considered patient facing and is a part of our Tuberculosis (TB) screening program. If selected for this role, you willbe requiredto be screened for TB.
Preferred Qualifications
+ Experience managing Medicare Advantagepanelsof patients with understanding ofbestpractice incoordinatedcare environment in a value-based relationship environment.
+ Knowledge of Medicare guidelines and coverage.
+ Knowledge of HEDIS quality indicators.
+ **Board certification in geriatric medicine is highly desirable.**

Pay Range
The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
$126,300 - $173,700 per year
This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
