Home Health / Hospice Aide
Join **Saint Agnes Home Health and Hospice** , part of Trinity Health At Home, and make a meaningful impact by providing compassionate care to clients in their homes.
Responsibilities:
+ Assist with personal care (bathing, grooming, dressing, mobility)
+ Light housekeeping, meal prep, and laundry
+ Monitor vital signs and report changes
+ Support medication self-administration and equipment safety
+ Maintain accurate documentation and communicate with RN supervisor
Qualifications:
+ High school diploma/GED
+ Must be a **Certified Home Health Aide (HHA) in the state of California**
+ Federally compliant training and competency evaluation
+ 1+ year experience in home care, hospice, or institutional setting
+ Driver’s license and reliable transportation
+ Ability to lift up to 35 lbs
