Home Health Physical Therapist

Part-Time | 2 Days per Week (16h) | Flexible Schedule

The Role

As a Part-Time Home Health Physical Therapist, you’ll provide individualized therapy services to patients in their homes while maintaining a flexible, predictable two-day schedule. You’ll focus on quality care, meaningful patient relationships, and measurable outcomes—without the pressure of a full-time caseload.

What You’ll Do

+ Complete in-home physical therapy evaluations and functional assessments

+ Develop and carry out personalized treatment plans based on patient needs and goals

+ Provide hands-on therapy to improve strength, mobility, balance, and safety

+ Monitor patient progress and adjust care plans as appropriate

+ Collaborate with physicians, nurses, and interdisciplinary care team members

+ Educate patients and caregivers on exercises, safe movement, and fall prevention

+ Maintain timely, accurate clinical documentation

+ Support continuity of care and positive patient outcomes

What You Bring

+ Graduate of an APTA-accredited Physical Therapy program

+ Active, unrestricted Physical Therapist license in the state of practice

+ Minimum of one year of clinical experience (home health experience preferred)

+ Strong clinical judgment and comfort working independently

+ Reliable transportation, valid driver’s license, and current auto insurance

+ Ability to travel within the assigned territory

+ Physical ability to assist, lift, and transfer patients (up to 50–100 lbs)
