    Job Summary

    The Clinical Care Leader (CCL), Home and Community Based Services (HCBS), is a remote registered nurse within the centralized intake team responsible for applying the nursing process to clinically validate home health and hospice referrals. This role ensures all required clinical documentation, eligibility criteria, and regulatory elements are complete and appropriate prior to referral handoff to the agency for timely admission. Collaborates with interprofessional colleagues within the centralized intake model to clinically review, validate, and coordinate home health and hospice referrals. Applies strong clinical judgment and the nursing process to assess referral documentation, confirm eligibility, and ensure regulatory and payer requirements are met prior to referral handoff for admission. Documents clinical review, findings, and determinations accurately and timely in designated electronic systems in compliance with applicable standards and regulations.

     

    Utilizes technology and electronic medical record systems to support safe, timely, and high-quality transitions from referral to admission. Serves as a clinical resource to intake staff and agency partners, supporting clarification of clinical information, physician orders, diagnoses, medications, and care needs to ensure referrals are admission-ready. Coordinates with physicians, referral sources, and agency teams to resolve gaps, discrepancies, or barriers impacting timely initiation of care.

     

    Maintains accountability for nursing oversight related to referral validation, clinical triage, and prioritization. Demonstrates strong clinical reasoning, critical thinking, and problem-solving skills to analyze referral data and respond appropriately to changing or complex clinical situations. Supports care coordination and patient education needs as appropriate through communication with referral sources, agencies, patients, and caregivers. Functions within the scopes and standards of nursing practice as outlined in the Nurse Practice Act and Administrative Rules in state of practice and licensure. The Sanford Professional Nursing Practice recognizes the Scope and Standards of Practice and the Code of Ethics for Nurses with Interpretive Statements as published by the American Nurses Association as the foundation of nursing care delivery and professional conduct.

    Qualifications

    Bachelor's degree in Nursing strongly preferred. Graduate from an accredited nursing program, including, but not limited to, those accredited by the Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), or National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA).

     

    Minimum of two years’ clinical experience preferred.

     

    Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state. Obtains and subsequently maintains required department specific competencies and certifications. Depending on location, a valid driver's license may be required. Basic Life Support (BLS) required. Certificate for Outcome and Assessment Information Set (OASIS). Certificate for OASIS Specialist - Clinical (COS-C) preferred.
