    Job Description:

    **Position Summary:** Assist the Access and Quality manager in day to day activities to ensure hospice referrals are processed timely and accurately. Implements workflows based on agency processes and policy. Partners with referral sources to provide customer support and to ensure all incoming referrals are appropriately coordinated. Supports department staff with day to day tasks and provides professional guidance

    Duties/Responsibilities:

    + Collaborates with Central Hospice intake specialist and coordinators, supports daily intake processes to ensure new patient referrals are retrieved from intake portals, faxes, phone and email, and entered in the home health EMR timely and accurately.

    + Works in conjunction with the Access and Quality manager in evaluating referral appropriateness

    + Demonstrates a solid understanding of the Medicare, Medicaid, and Private insurance environment and promotes coordination with the Business Office to identify and verify patient’s insurance.

    + Guides workflow processes to ensure accepted referrals are transitioned from intake to insurance verification/authorization and to scheduling timely and accurately in order to prevent delays in patient admission to service.

    + Responds promptly and courteously to all patient inquiries or problems.

    + Mentors associates and provides input into performance evaluations.

    + Works to facilitate individual and team development that drives positive results.

    + Responds to all requests in a positive manner.

    + Elicits and gathers accurate and pertinent information regarding the patient’s medical, psycho-social and environmental condition to accurately identify critical information needed to initiate the plan of care.

    + Working knowledge of the skills and service provided by each member of the multidisciplinary team and the ability to relate these to identified patient needs

    + Prioritizes referrals based on patient’s needs and referral requests.

    + Understands and integrates relevant hospice standards, regulations and practices to make decisions regarding appropriateness for care.

    + Accepts and documents verbal orders related to the initiation of services. Inputs initial information into the computer to begin the admission process.

    + Reviews all admission medications to ensure correct coverage in Enclara

    + Reviews all admission narratives for accuracy

    + Forwards all referral information to the appropriate team for timely initiation of services.

    + Responds to telephone requests for information and interprets services to callers.

    + Uses information technologies including E-Discharge, E-mail, and Epic EMR to process and track referrals and communicate with team members.

    + Develops positive relationships for new and existing referral sources, including physician offices, hospitals and rehab facilities.

    + Collaborates with hospital case managers in outreach efforts.

    + Acts as a resource to referrers, staff, families and patients to develop solutions for excellent patient care.

    + Develops and maintains relationships with referrers through telephonic contact and occasional meetings to ensure quality services.

    _Communication_

    + Communicates with the physician regarding the patient’s needs and obtains/receives physician’s orders as required.

    + Attends regular agency and team meetings as needed.

    + Provides timely communication to Clinical Managers whenever there are unusual requests or complex situations/orders.

    + Communicates with other disciplines on the care team and community providers to coordinate the care plan (i.e. Insurance case managers, high risk case managers, elder services, protective service, etc).

    + Practices confidentiality principles set by the agency and federal HIPAA guidelines.

    + Documentation-Must have attention to detail when taking, processing and documenting referral requests throughout the intake process.

    + Proficiency with common office software programs and ability to learn other software products including, but not limited to Epic and E-Discharge.

    + Completes documentation in accordance with agency timeliness policies.

    + Professional Development: Maintains and updates clinical knowledge and skills based on current nursing practice.

    + Adheres to departmental requirements for required training.

    + Identifies specific learning needs and goals and collaborates with nursing leadership in developing a plan to meet them.

    + Participates in in-service programs; teaches and precepts other Homecare Intake Coordinator personnel.

    Education/Experience Required:

    + Professional Nurse from a Graduate of National League for Nursing accredited school of nursing.

    + 1-2 years’ supervisory experience in Hospice. 3-5 years of oncology or hospice experience

    + Comprehensive knowledge of hospice services.

    + Knowledge of Medicare and Medicaid regulations in Hospice

    + Demonstrates excellent observation and problem solving/critical thinking skills.

    + Well-developed verbal and written communication skills; essential computer skills to work with EMR.

    + Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.

    + Self-directed and able to work with minimal supervision.

    + Shows strong ability to organize and prioritize workload independently.

    Registration/Certification:

    + Registered Nurse or Licensed Practical Nurse licensed in the state of MA

    + Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.

     

    Physical/Environmental Requirements

     

    This job requires frequent bending, carrying, pulling, pushing, reaching, standing, stooping, walking and lifting over 10 lbs. There will be occasional need for climbing stairs, lifting in excess of 50 – 100 lbs and sitting. Constant use of hearing, sight and speech. Frequent use of smell and touch. Frequent keyboard and telephone use. Constant concentration and problem solving.

    Pay Range:

    $37.00 - $80.00

     

    The pay range listed for this position is the base hourly wage range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law. Compensation may exceed the base hourly rate depending on shift differentials, call pay, premium pay, overtime pay, and other additional pay practices, as applicable to the position and in accordance with the law.
