• Residential Case Manager

    Salary Range $24.00 - $26.00 Hourly

    Description

    As a Case Manager, you will support clients in the Residential System and perform duties in collaboration with a clinician. You will report to the Case Management Supervisor, Residential and you will work onsite with community-based travel..

    Responsibilities:

    + Arrange and attend home visits

    + Prepare transition plans

    + Coordinate Independent Living Services

    + Be the primary contact between Schools and Residential

    + Coach family visits

    + Complete various assessments, including FASP and SPR

    + Complete accurate case notes and input the notes into Connections database

    + Provide transportation for clients

    + Maintain contact with DSS and family

    + Function as a contributing member of the interdisciplinary treatment team

    + Attend dorm meetings

    + Attend treatment review meetings

    Requirements and Education:

    + Requires a HS Diploma/ GED

    + Experience with individuals in care systems preferred.

    + Must pass physical.
