    $5,000 Sign-On Bonus for eligible new part-time and full-time employees joining UW Medicine

     

    The UW Medical Center-Montlake Social Work Department** has an outstanding opportunity for a **Social Worker - Cystic Fibrosis Clinic.

    WORK SCHEDULE

    + Part-Time / 32 hours per week

    + Day Shifts (Monday/Thursday on-site and 2 day telework)

    + **This position is expected to last through 6/30/2026 to be renewed annually upon notice of funding**

    POSITION HIGHLIGHTS

    + The Cystic Fibrosis (CF) Social Worker provides expertise in helping patients and families with their emotional and practical needs and supports patients and support systems in coping with CF in the different impacts of their chronic condition, treatment, and involved in the transition stages, including transition to adult care and across lifespan

    + Be responsible for providing a full range of social work services including psychosocial assessment, treatment planning, therapeutic interventions, crisis intervention and resource referral(s)

    + Partner with the patient and the health care team to coordinate the patients’ care through effective utilization of resources

    PRIMARY JOB RESPONSIBILITIES

    + Complete annual CF Social Work screening for health-related social needs and social drivers of health (SDOH) such as housing instability/ homelessness, transportation, food insecurity, unreliable access to medications, and insurance.

    + Provide specialty consultations to other health care staff and health care teams regarding psychosocially focused aspects of patient care that contribute to barriers to care.

    + Provide psychosocial support and assistance with care navigation to patients with complicated social and medical histories, specifically around disability.

    + In collaboration with the multidisciplinary team, develop and implement safe and timely interventions to address access to care and services.

    + Coordination of care also includes consideration of other aspects of comorbidities, including mental and behavioral health conditions, chronic pain, substance use, sleep, executive functioning challenges, body image concerns, disordered eating, and end of life concerns.

    + Alignment and continuous education with CF patient associations, attend national conference, and remain current on the evolving disability needs of individuals with CF.

    REQUIRED POSITION QUALIFICATIONS

    A Master of Social Work Degree from a program accredited by the Council on Social Work Education AND one year of full-time social work experience in a health care setting or equivalent. Experience can include a practicum placement as part of a social work training program and/or employment experience.

    LEGAL REQUIREMENT

    Within 90 days of hire must hold one of the following credentials issued by the state of Washington:

    • Agency Affiliated Counselor

    • Licensed Social Work Associate -Advanced

    • Licensed Social Work Associate - Independent Clinical

    • Licensed Advance Social Worker

    • Licensed Independent Clinical Social Worker

    Compensation, Benefits and Position Details

    Pay Range Minimum:

    $88,632.00 annual

    Pay Range Maximum:

    $138,720.00 annual

    Other Compensation:

    $5,000 Sign-On Bonus

    Certification Premium - $1.50/hour
