    **Facility:** BayCare Health System, Rehab Services Main – SJHN

    **Location:** St. Joseph’s North - Lutz, FL

    **Status:** Pool (Non-Benefit Eligible)

    **Exempt:** No

    **Shift Hours:** Sundays 8:30 AM – 5:00 PM; weekdays as needed

    **Shift:** Days

    **Weekend Work:** Every Other

    **On Call:** No

    Physical Therapist (Pool) Responsibilities:

    + Perform comprehensive physical therapy evaluations and develop individualized treatment plans.

    + Deliver evidence-based interventions to improve mobility, strength, balance, and functional independence.

    + Collaborate with physicians, nurses, case managers, and other therapists to coordinate care.

    + Educate patients and families on exercises, safety, and post-discharge recommendations.

    + Document all patient care activities accurately and timely in the electronic medical record.

    + Participate in team meetings and contribute to quality improvement initiatives.

    Requirements:

    + **Required:** BLS (Basic Life Support)

    + **Required:** PT (Physical Therapy) License

    + OR PT (Physical Therapy) – Eligible to Obtain

    + OR PT (Physical Therapy) – Temporary, with licensure within 6 months

    + **Required:** Bachelor’s Degree in Physical Therapy

    + **Preferred:** 1 year of Physical Therapist experience
