Job Description:

+ Performs behavioral health intakes, psycho-social assessments, care plans, and psychotherapy with patients and families coping with behavioral health disorders.

+ Maintains comprehensive community resources and networks to assist patients and families as they transition back to the community.

+ Collaborates with the multi-disciplinary team in establishing and documenting psycho-social determinants of health to attain safe discharges and avoid emergency hospital admissions.

+ Serves as a consultant and educator to the multi-disciplinary team regarding psycho-social components of illness and hospitalization.

+ Maintains professional competency by participating in continuing education and appropriate learning experiences, both internally and externally.

Master's (Required)Basic Life Support - CPR Cert (BLS) - RQI Resuscitation Quality Improvement, Cardiopulmonary Resuscitation (CPR) - Accredited Issuing Body, Clinical Social Worker License (LCSW) - EV Accredited Issuing Body, Licensed Mental Health Counselor (LMHC) - EV Accredited Issuing Body

Pay Range:

$26.08 - $48.50
