Job Brief
The primary role of the Community Habilitation Specialist is to work alongside the people we support and provide Medicaid Waiver Services in community-based settings in order to help them to achieve their Personal Outcomes.

Position Duties
include, but are not limited to, the following:
+ Work with individuals with intellectual and developmental disabilities to develop daily living, vocational, and decision-making skills while supporting their overall health and safety.
+ Provide training in accordance with valued outcomes outlined in each individual’s Staff Action Plan and Life Plan.
+ Deliver training and supervision based on each participant’s abilities, including self-care, independent living, community integration, and work-related skill development.
+ Ensure the safety and security of individuals receiving services by considering their specific needs and limitations and providing support in the least restrictive manner.
+ Support individuals in working toward Person-Centered Plan (PCP) goals to promote increased independence.
+ Complete all required documentation related to service delivery in compliance with federal and state regulations.
+ Provide services in a timely manner according to each participant’s assigned schedule to ensure consistency and respect for individuals served.
+ Work collaboratively with interdisciplinary team members to support participant outcomes.
+ Respond appropriately to crisis and emergency situations to ensure the safety and well-being of participants.
+ Promote program development by actively identifying and engaging individuals who may benefit from services.
+ Maintain effective interpersonal, verbal, and written communication skills and demonstrate proficiency in personal computer use, including Microsoft Word.
+ Travel throughout Putnam County and Southern Dutchess County as assigned.
+ Perform all duties in full compliance with applicable laws, rules, regulations, policies, and procedures.
+ Adhere to the agency’s Corporate Compliance Program, including the Code of Business Conduct, New York State and federal False Claims Acts, whistleblower protections, and mandatory reporting requirements.
+ Complete corporate compliance training at least annually and fully cooperates with all compliance investigations and reviews.
+ Monitor and respond appropriately to changes in individuals’ physical or emotional condition.
+ Participate with the trans-disciplinary team in the development, implementation, and evaluation of individualized program plans.
+ Assist in the evaluation of individual skills.
+ Perform all other duties as assigned.
Requirements Education
+ High School Diploma or GED preferred.
Experience
+ Minimum of two (2) years of experience working with individuals with developmental disabilities preferred.
License
+ Valid New York State Driver’s License.
+ Must meet agency requirements for insurability.
