    Responsibilities:

    + Assist in the development, review, and revision of the plan of care.

    + Provide nursing care per the physician’s plan of treatment, individualized plan of care, as authorized by the client’s payer source.

    + Observe, record, and report any changes in the client's condition to appropriate personnel and/or physician.

    + Deliver ongoing instruction and supervision to clients, families, and health team personnel in the plan of treatment.

    + Monitor and document the client's condition and provided care services using electronic charting.

    + Participate in education programs, quality assessment activities, nurse meetings, and other staff meetings as required.

    Requirements:

    + Active Licensed Practical Nurse or Registered Nurse License

    + Strong problem-solving skills and ability to multi-task

    + Excellent communication skills

    + Positive and professional attitude
