Required Qualifications (as evidenced by an attached resume):
Bachelor’s degree (foreign equivalent or higher) in Nursing or related fields. A valid New York State Registered Professional Nurse License. Two (2) years of full-time case management experience.
Preferred Qualifications:
Additional three years (3) of full-time case management experience. Two (2) years full-time nursing experience in either a hospital, ambulatory or medical office setting. Experience working with patients with co-morbid chronic illnesses, including cancer, dementia and transplants. Mental health experience, particularly in post-traumatic stress or depression. Experience working with hospitals, long-term care facilities and hospices. Experience utilizing community resources, including transportation services. Durable medical equipment experience. Experience with developing and implementing treatment plans or Plan of Care notes and conducting assessments for case management services. Experience working with 9/11 Responders and their families. Experience with Electronic Medical Records systems. Proficiency with Microsoft Office.

The Stony Brook WTC Health Program offers comprehensive, integrative healthcare of 9/11-related illnesses for WTC disaster responders. Integral to our mission of patient-centered care is an enduring support community, advocacy for our 9/11 responders, and cutting-edge research efforts. The RN - Case Manager must possess the knowledge and experience to assess, plan and intervene as needed, to promote health, secure services and help patients cope with serious health issues. The incumbent must be able to work independently and as a member of a team. The successful incumbent must be detail oriented and have strong oral and written communication skills. The Case Manager will be responsible for the following duties:
+ Responsible for care/case management, primarily for World Trade Center Health Program (WTCHP) patients with intensive and progressive case management needs for cancer care, dementia care, end of life, surgeries including transplants for a population that has complex aero digestive health conditions, mental health conditions, and/or cancer due to their WTC exposures.
+ Assess the patient’s acuity and needs for CM, including certified conditions, other conditions, disabilities, barriers to care, and overall ability to manage and comply with treatment program.
+ Educate patients re: WTC Health Program benefits and limitations.
+ Perform appropriate referral and disposition planning.
+ Track and monitor hospitalizations and ensure safe discharge plan and pre-authorizations.
+ Coordinate with service providers to pre-authorize covered tests, procedures and treatments.
+ Provide support services to patients and families. Work collaboratively with social workers, providers, pharmacists, and others to ensure wrap-around care for the members.
+ Ensure professional, detailed records for all interactions according to Program requirements. Assist in gathering records for government audits related to cases in caseload.
+ Assist Supervisor and team with the continuous improvements to the care management program. This includes special projects and workgroups that serve to assist our population.
+ Other duties as assigned.

The Research Foundation of SUNY is a private educational corporation. Employment is subject to the Research Foundation policies and procedures, sponsor guidelines and the availability of funding. FLSA Exempt position, not eligible for the overtime provisions of the FLSA. Minimum salary threshold must be met to maintain FLSA exemption.
The incumbent must be willing to work and travel between the Westbury and Commack clinic locations. Occasional Evenings and Saturdays will be required.
