    IDD (Intellectual Developmental Disabilities) Care Coordinator (Community-Based)

    Schedule: Full time | Mon–Fri | 9a–5p (flexible)

    Start: Feb 2026 (approx. 2 weeks from offer)

     

    Contract: 13 weeks | Extensions common | Temp-to-Perm supported

    Pay: $32/hr

    OT: 1.5× after 40 hrs

     

    Mileage: Reimbursed at IRS standard rate

    Overview

    Amergis Healthcare Staffing is hiring an IDD Care Coordinator to support community-based behavioral health programs in Oakland or Macomb County, MI. This position focuses on supporting individuals with intellectual and developmental disabilities through person-centered planning, community resource linkage, family support, and empowerment-focused interventions.

     

    HCPs may select Oakland or Macomb as their preferred county. The role includes floating between multiple program sites and completing community visits.

    Position Requirements

    + Bachelor’s degree in Human Services, Psychology, Social Work, or related human-services field

    + Valid Michigan driver’s license

    + Experience supporting individuals with IDD preferred

    + Strong documentation, organization, and communication skills

    + Comfort with community-based work and travel

    Key Responsibilities

    + Develop, update, and monitor individual treatment and support plans

    + Coordinate Person-Centered Planning and evaluate progress and satisfaction

    + Support clients with communication, self-advocacy, daily skills, and choice-based decision making

    + Provide education, guidance, and empowerment-focused interventions

    + Train caregivers to meet client needs and preferences

    + Assist individuals in accessing community services, natural supports, and health care providers

    + Maintain contact with family members and key supports to ensure continuity of care

    + Document services in compliance with agency and Medicaid Provider Manual guidelines
