    Job Description:

    Participates in staff and department meetings, in-services, and educational seminars. Travels to patient locations to facilitate hospice service enrollments. Explains hospice services to potential patients and families, addressing fears and physical limitations. Discusses financial obligations and insurance coverage benefits, including Medicare. Obtains necessary patient consent and admission forms, such as the Medicare Election of Benefit. Other duties as assigned. Identifies and addresses objections, concerns, or entry barriers by providing relevant information. Enters information into Electronic Medical Record and completes necessary documentation. Communicates immediate care needs to hospice referral/admissions manager. Coordinates with multiple departments for timely admission following patient discharge. Accepts assignments from staff referrals. Completes required documentation thoroughly and provides accurate information related to insurance, medical group, and demographics.

    Pay Range:

    $19.23 - $30.77
