    Job Description:

    + Ensures compliance with Disease Specific standards of care and other regulatory bodies.

    + Participates in conferences and teleconferences for Readmission Reduction programs. Compiles data for audits and accreditation.

    + Coordinates patient care post-discharge for high-risk readmission patients.

    + Stays updated on regulatory requirements. Engages in discussions to reduce readmissions. Gathers and reports data accurately for audits.

    + Manages follow-up care to prevent readmissions. Develops and implements strategies to reduce readmissions.

    QUALIFICATION REQUIREMENTS:

    Bachelor's (Required), Technical/Vocational School (Required)Advanced Cardiac Life Support Cert (ACLS) - RQI Resuscitation Quality Improvement, Advanced Practice Registered Nurse (APRN) - EV Accredited Issuing Body, Basic Life Support - CPR Cert (BLS) - RQI Resuscitation Quality Improvement

    Pay Range:

    $68,132.50 - $119,520.35
