    Job Description:

    Ensures compliance with Disease Specific standards of care and other regulatory bodies. Participates in conferences and teleconferences for Readmission Reduction programs. Compiles data for audits and accreditation. Coordinates patient care post-discharge for high-risk readmission patients. Stays updated on regulatory requirements. Engages in discussions to reduce readmissions. Gathers and reports data accurately for audits. Manages follow-up care to prevent readmissions. Develops and implements strategies to reduce readmissions. Collaborates with interdisciplinary teams to improve patient outcomes. Monitors and evaluates the effectiveness of readmission reduction initiatives. Provides education and support to patients and families to prevent readmissions. Other duties as assigned.

    QUALIFICATION REQUIREMENTS:

    Bachelor's (Required), Technical/Vocational School (Required)Advanced Cardiac Life Support Cert (ACLS) - RQI Resuscitation Quality Improvement, Advanced Practice Registered Nurse (APRN) - EV Accredited Issuing Body, Basic Life Support - CPR Cert (BLS) - RQI Resuscitation Quality Improvement

    Pay Range:

    $68,132.50 - $169,845.73
