    Job Description:

    + Promotes excellent customer services through interactions with patients, physicians, care managers, and the healthcare team.

    + Collaborates with the Care Managers for patient care coordination needs.

    + Coordinates patient care and services arrangements under the direction of a RN or SW Care Manager and in collaboration with the patient/family.

    + Provides and documents the delivery of regulatory letters to patients or their representatives in accordance with Federal and State regulations.

    + Arranges patient transportation and follow up appointments as requested.

    QUALIFICATION REQUIREMENTS:

    High School Grad or Equiv (Required)

    Pay Range:

    $17.11 - $27.38
