Job Description:

+ Conduct comprehensive patient assessments to gather data on physical, emotional, and social health status.

+ Develop and implement individualized care plans based on assessment findings, setting goals and outlining interventions.

+ Administer medications and treatments accurately as prescribed, ensuring patient safety and adherence to protocols.

+ Monitor and evaluate patient responses to interventions, adjusting care plans as necessary and documenting progress in the electronic medical record.

+ Collaborate with multidisciplinary teams to resolve patient care issues and ensure coordinated, comprehensive care.
