Job Description:

$3,000 Relocation Assistance (*Terms: 12 month Employment Contract Required for Relocation; Relocation Eligibility: Must live 50 miles outside of campus address.)

+ Evaluates the psychosocial needs of patients and families, developing tailored care plans that address psychological, social, functional, and financial impacts of illness.

+ Travels to patient residences for initial assessments, identifying and assessing psychosocial needs and understanding caregiving dynamics.

+ Assists patients and families in obtaining financial resources and provides education and assistance in completing Advance Directives.

+ Offers individual or group counseling to patients and families, creating and implementing individualized discharge plans according to standards of practice and patient needs.

+ Communicates effectively with caregivers, making referrals for intra-organizational services and community resources such as Medicaid, Salvation Army, and food banks.

Bachelor's (Required), Master's (Required)Auto Insurance (AL) - EV Accredited Issuing Body, Basic Life Support - CPR Cert (BLS) - RQI Resuscitation Quality Improvement, Driver's License (DL) - EV Accredited Issuing Body, Licensed Social Worker (LSW) - EV Accredited Issuing Body

Pay Range:

$23.71 - $44.09
