    Job Description:

    Relocation Bonus: $1,500

     

    Schedule: Full Time 40 Hours Per Week Mon-Fri, with Weekend Rotation. Every 2 to 4 Shifts per month staffing in operational role.

    Shift: Days

    + Participates in departmental and institutional committees, representing pharmacy services and contributing to performance improvement projects.

    + Coordinates Clinical Pharmacy Program Development and Quality Initiatives- Designs and implements pharmacy programs, quality improvement efforts, and medication stewardship initiatives to enhance medication-use systems and patient outcomes.

    + Facilitates Formulary Management and Clinical Services- Develops treatment guidelines, formulary recommendations and protocols.

    + Supports P&T Committee preparation and provides consultation on therapeutic appropriateness.

    + Directs Quality Assurance and Performance Improvement- Develops and monitors quality metrics, leads improvement initiatives and benchmarks performance against internal and external standards.

    Bachelor's (Required)Licensed Pharmacist (RPh) - EV Accredited Issuing Body

    Pay Range:

    $105,006.04 - $195,321.62
