    Job Description:

    + Educates members on their care team and assists with coordination of care across the continuum, including scheduling appointments and referrals.

    + Participates in daily operations of the practice, including answering phone calls, assisting with front desk inquiries, and maintaining the clinical space. Communicates through various digital channels to support patient care.

    + Performs initial health screens and intake for new members. Obtains and reviews old medical records to ensure comprehensive patient history.

    + Identifies care gaps and orders necessary screening tests. Confirms appointments and provides instructions to patients.

    + Conducts pre-visit RAF reviews to prepare for patient visits. Welcomes patients, provides tours of the practice, and reviews welcome packets. Ensures all medical records are available for office visits.

    QUALIFICATION REQUIREMENTS:

    Technical/Vocational School (Required)Basic Life Support - CPR Cert (BLS) - RQI Resuscitation Quality Improvement, Certified Clinical Medical Assistant (CCMA) - EV Accredited Issuing Body, Certified Medical Assistant (CMA) - EV Accredited Issuing Body, Clinical Medical Assistant Certification (CMAC) - EV Accredited Issuing Body, National Certified Medical Assistant (NCMA) - EV Accredited Issuing Body, Nationally Registered Certified Medical Assistant (NRCMA) - EV Accredited Issuing Body, Registered Medical Assistant (RMA) - EV Accredited Issuing Body

    Pay Range:

    $17.11 - $27.38
