Description:
Position Purpose
The Team Lead supports daily operations and ensures quality care for individuals served. This role focuses on guiding staff, maintaining compliance with policies, and driving performance to meet key benchmarks through strong leadership and clear communication.
What You Will Do
+ **Care Planning:** Develop individualized care plans with residents, families, and interdisciplinary teams.
+ **Support & Advocacy:** Provide emotional/behavioral support and advocate for residents with families, agencies, and healthcare professionals.
+ **Staff Training & Oversight:** Train staff in medication administration and other required areas; assist with onboarding and performance feedback.
+ **Compliance & Safety:** Monitor policy implementation, conduct certified investigations, and ensure a safe environment.
+ **Program Management:** Make person-centered decisions and develop effective action plans for program success.
Minimum Qualifications
+ Master’s degree + 1 year experience with individuals with intellectual disabilities/autism **OR** Bachelor’s degree + 2 years’ experience **OR** Associate degree/60 credits + 4 years’ experience
+ Valid driver’s license and ability to drive agency vehicle
