• LPN Case Manager

    Amergis (Braintree, MA)


    Amergis Healthcare is seeking an LPN In Home Case Manager to provide admissions assessments in Cambridge and the surrounding area. In this fulfilling role, you will provide community-based care services designed to help elderly and disabled individuals remain safely and comfortably in their homes.

     

    Local Pay: $47/hour + mileage reimbursed at $0.70/mile

    Qualifications:

    + Active, unrestricted MA nursing license

     

    Schedule: Monday–Friday, 9:00 AM–5:00 PM

    Job Duties:

    + Complete intake, annual assessments, and admission visits.

    + Complete all required documentation, including client assessments and multidisciplinary plans of care; coordinate with PCPs and other healthcare providers.

    + Review and educate caregivers on admission binder materials, communication tools, and/or EVV systems.

    + Conduct regular in-home visits, provide caregiver skills training, and assess for health or environmental changes.

    + Conduct semi-annual visits, updating care plans, goals, medication reviews and reconciliations, and caregiver communication tool audits.

    + Identify concerns related to safety, psychosocial status, and physical health.

     

    Duration: 13 weeks with possible extension
