SHH Licensed Practical Nurse (LPN) – Full Time

Your Role as a Licensed Practical Nurse

As an LPN with Elara Caring, you’ll provide skilled nursing care under the direction of a Registered Nurse, ensuring patients receive safe, compassionate, and effective care in their homes.

You will:

+ Deliver skilled nursing care in accordance with the patient’s Plan of Care, physician orders, and established standards

+ Support patient goals through high-quality, individualized nursing visits

+ Communicate changes in patient condition to the RN Case Manager, physician, and care team

+ Promote patient safety and report unsafe conditions promptly

+ Educate patients and caregivers on self-care techniques

+ Assist with medication management and accurate documentation

+ Collaborate closely with the RN Case Manager for guidance and support

+ Help prevent avoidable hospitalizations and emergency room visits

+ Treat patients, families, and caregivers with respect, compassion, and professionalism

What We’re Looking For

+ Graduate of an accredited nursing program (Certificate, Diploma, or Associate Degree)

+ Active **State LPN/LVN license**

+ At least **1 year of clinical nursing experience**

+ Hospice or home health experience preferred

+ Ability to travel locally (approximately 50%)

+ Valid driver’s license, reliable transportation, and insurance

+ Ability to meet physical demands of the role, including lifting up to 50–100 lbs
