    Description

    The primary purpose of the TRC Care Manage is to serve as a role model and community liaison to clients who are enrolled in the Trauma Recovery Center that serves the victims of violent crimes and their families towards healing and health of symptoms related to the trauma from the violence.

     

    TRC Care Managers are expected to develop relationships based on trust with clients and families utilizing a trauma-informed approach, provide reliable information, appropriate community resource referrals, and emotional support to victims of violence. The TRC Care Manager will meet with clients in the community, as well as structured and assertive outreach meetings to referral resources and community supports. This TRC Care Manager will function as a core member of the TRC treatment team.

    Job Responsibilities:

    + Assists in the development of a violence intervention program, with demonstrated outcomes in reducing trauma recidivism, targeting the underserved and low-income population.

    + Develops and establishes relationships with all victims of violence who are referred to Astor.

    + Provides the clients with emotional support as they recover from their traumatic event utilizing 1:1 outreach and meetings, individualized care plans, and comprehensive patient navigation.

    + Evaluates and assesses the client’s current community-based needs and barriers to meeting these needs. Collaborates with hospital social work to provide outreach services.

    + Provides the clients with referrals, encouragement, and resources to address the needs identified by the TRC- Care Manager and the trauma team, in collaboration with the TRC Program Coordinator.

    + Helps clients access services by assisting in scheduling appointments, reminding patients of appointments, and accompanying patients to appointments when necessary.

    + Visits clients at home or other locations when necessary, to ensure recommended services are being received. Actively supports and monitors client linkages with services.

    Requirements

    You must have:

    • Bachelor's degree required, BA in counseling, social work, psychology, forensics or related field preferred.

    • Must be bilingual (English/Spanish).

    • Successful completion of training including trauma informed care training, orientation to the Trauma Recovery Center mission and service, and shadowing of community outreach organizations.

    • Two (2) years of recent experience, such as any combination of student, volunteer, or work experience, which involves the Bronx area and its community resources, is required.

    • Participation in community organizations and firsthand understanding of issues related to interpersonal violence preferred.

    • Must have a basic understanding of computer use and software applications, and the ability to perform basic internet searches Must have demonstrated interest in serving the Bronx community.

    • Must be able to work independently and seek guidance when necessary.

    • Must have good listening skills.

    • Must have a demonstrated ability to work in a team environment and have the ability to work with individuals of a diverse background.

    • Must have valid driver's license or ability to use public transportation to accompany clients to appointments and connect with community resources, as needed.

    You should have:

    • Sensitivity to a substance abuse environment preferred.

    • Previous mentoring experience is desired.

    Salary Description

    $51,000-$56,100/year
