    ### Job Duties

    Both first and second shifts are available on weekends.

     

    Position Summary: The hired individual will provide therapy, counseling and case management services within a social work framework, working with BTH guests on their behavioral and social health needs. This person will promote the optimal health of guests, and refer guests to additional care and support where appropriate.

    Essential Duties and Responsibilities:

    Obtain and accurately document a relevant behavioral health history from guests as needed.

     

    Care plan development, including initial assessment of behavioral and social needs.

     

    Care plan implementation follow-up and monitoring.

     

    Support and referral assistance for legal, housing, transportation, case management and community social services.

     

    Support and referral assistance for health care benefits, health and other eligible benefits.

     

    Provide social and behavioral services that can be provided in the recuperative care setting.

     

    Collaborate as a member of the guest’s interdisciplinary health team, as needed and desired by the guest.

     

    Document services provided consistent with Medicaid billing requirements.

     

    Collaborate with BTH Team Members to create a positive and healthy work environment.

     

    Support the creation of systems and development opportunities.

     

    Other duties as assigned within the scope of practice and to maintain and grow BTH.

     

    Engage in professional development in order to meet licensing requirements and ensure culturally responsive best practices are applied.

     

    Collaborate with community partners to enhance outcomes for BTH guests.

     

    ### Minimum Education Required

    Degree in social work.

     

    Meets DHS criteria to pass a background check

     

    Must obtain/maintain CPR or First Aid Certification within 6 months of hire

     

    Ability to work independently and in a collaborative work environment.

     

    Experience working with diverse communities.

     

    Strong listening, critical thinking, and problem-solving skills.

     

    The MSW will be required to abide by standard protocols for the protection of the confidentiality of information of guests.

    Since 2018, BTH has been offering hospitality to people experiencing homelessness who’ve had an acute medical condition, and are ready to leave the hospital, but need a safe place to continue to fully heal. BTH serves individuals in Duluth, MN and the surrounding region. While guests stay with us, we work on supporting social determinants of health, housing and connections to supports in the community in hopes that they'll be in a better position when they leave our home.
