    Job Description:

    Evaluates the psychosocial needs of patients and their families. Develops and implements psychosocial care plans based on patient and family needs. Conducts initial assessments at patient residences. Assists patients and families in obtaining financial resources. Provides services as determined by the interdisciplinary team and report to the registered nurse. Communicates with caregivers and makes referrals to community resources. Educates and assists patients and families in completing Advance Directives. Offers individual or group counseling to patients and families. Interacts with facility/hospital case managers to coordinate hospice services. Assesses and documents family bereavement needs from admission through the hospice process. Maintains professional documentation in the electronic medical record. Other duties as assigned.

    QUALIFICATION REQUIREMENTS:

    Bachelor's (Required), Master's (Required)Auto Insurance (AL) - EV Accredited Issuing Body, Basic Life Support - CPR Cert (BLS) - RQI Resuscitation Quality Improvement, Clinical Social Worker License (LCSW) - EV Accredited Issuing Body, Driver's License (DL) - EV Accredited Issuing Body

    Pay Range:

    $23.71 - $44.09
